Catholic Health Initiatives Expands Patient Engagement Focus With Allscripts
CHI, one of the nation’s largest nonprofit health systems, will expand its use of the FollowMyHealth patient engagement platform from the ambulatory to acute care settings, replacing one of its legacy acute patient portals, to provide patients a single portal experience. CHI implemented FollowMyHealth across a number of ambulatory settings in 2015. With its expanded use of FollowMyHealth, CHI will build upon the organization’s success in achieving high patient-adoption rates.
“Allscripts FollowMyHealth was immediately successful for our organization, with 46,000 patients enrolling in a just few months,” Dr.
CHI will also implement FollowMyHealth Achieve, which enables providers to involve patients directly in the ongoing management of their care. With FollowMyHealth Achieve, clinicians enter orders in the Allscripts TouchWorks® electronic health record (EHR), which are then available to patients at home or on their mobile device through the portal. Home-based wireless technologies capture readings and results, and push the information to the EHR. The care team is automatically notified if the patient is noncompliant or if individual results exceed clinician-established parameters. FollowMyHealth Achieve enables providers to monitor conditions remotely and, more importantly, initiate interventions as needed to influence behavior and impact outcomes.
